Statement of Information Practices
(Personal Health Information Protection Act, 2004)

1. Our Organization

 

Imagus Inc., operating as Charlton X‑Ray & Ultrasound and Ancaster X‑Ray & Ultrasound, is a health information custodian within the meaning of the Personal Health Information Protection Act, 2004 (PHIPA).

Imagus Inc. operates licensed Diagnostic Imaging facilities in Ontario under the Integrated Community Health Services Centres Act (ICHSC Act) and provides insured diagnostic imaging services in accordance with applicable provincial legislation and regulations.

2. Personal Health Information We Collect

In the course of providing diagnostic imaging services, Imagus Inc. collects personal health information about patients, which may include:

  • Patient identifiers, such as name, date of birth, and health card number

  • Diagnostic imaging requisitions and referral information

  • Diagnostic images (including X‑ray and ultrasound images)

  • Diagnostic imaging reports prepared by radiologists

  • Appointment, scheduling, and related administrative information

Personal health information is collected directly from patients and from referring health‑care providers, as permitted or required by law.

3. Purposes for Collection, Use, and Disclosure

 

Imagus Inc. collects, uses, and discloses personal health information for purposes that include:

  • Providing diagnostic imaging services

  • Communicating diagnostic imaging results to referring physicians and other health‑care providers

  • Supporting continuity of care for patients

  • Conducting quality assurance, quality improvement, and accreditation activities

  • Meeting legal, regulatory, professional, and billing requirements

 

Personal health information is collected, used, and disclosed in accordance with PHIPA and other applicable legislation.

4. Electronic Systems and OCINet

Imagus Inc. uses electronic information systems to store and provide access to patients’ diagnostic imaging information.

Diagnostic imaging images and reports are uploaded to, stored in, and accessed through the Ontario Clinical Imaging Network (OCINet), including:

  • OCINet Diagnostic Imaging Repository (DI‑r)

  • OCINet Viewer

 

These systems are used to support the delivery of patient care and to facilitate appropriate access to diagnostic imaging information by authorized health‑care providers, in accordance with PHIPA and applicable agreements.

Imagus Inc.’s diagnostic imaging systems interface with OCINet. The clinic uses Velox Medical Systems as its Picture Archiving and Communication System (PACS) and Radiology Information System (RIS). Velox is a web‑based system, and diagnostic images are stored within Velox’s secure data centre environment.

5. Agents and Service Providers

 

Imagus Inc. may use external service providers (agents) to assist in providing diagnostic imaging services and supporting clinic operations, including diagnostic imaging system vendors and information technology service providers.

Imagus Inc. contracts with IT Business Advisors for information technology support. IT Business Advisors does not interact with or access personal health information.

All agents are required to protect personal health information and may only use or disclose it as permitted by Imagus Inc. and by law.

6. Safeguards

 

Imagus Inc. takes reasonable steps to protect personal health information against theft, loss, and unauthorized use or disclosure.

Safeguards include:

  • Administrative safeguards, such as privacy policies, procedures, and staff training

  • Technical safeguards, such as access controls and secure electronic systems

  • Physical safeguards, such as restricted access to clinic areas and records

 

Safeguards are appropriate to the sensitivity of the personal health information.

7. Access, Correction, and Complaints

Patients have the right to request access to or correction of their personal health information, subject to limited exceptions under PHIPA.

Questions or concerns about Imagus Inc.’s privacy practices, or requests for access or correction, may be directed to the

The Privacy Officer is also responsible for managing and responding to privacy breaches.

Patients also have the right to make a complaint to the Information and Privacy Commissioner of Ontario if they believe their privacy rights have been violated.

8. Availability of This Statement

This Statement of Information Practices is available to the public on Imagus Inc.’s website and and in printed form upon request from the clinic receptionist.
